Welcome to ASA Charter School! Thank you for your interest in our school. We are very excited that you have chosen ASA Charter School for your child's education.
 
Parents/Guardians - Please submit your child's complete enrollment forms.  
 
New Students - If you are a NEW student, please complete the following forms:
- New Student Enrollment Form
- Student Service Questionnaire
- Housing Questionnaire
- Income Survey - not yet available
- Internet Access and Network Use Agreement
- Media Consent and Security Notice
- Document Upload 2024-2025 for any documents for your student's file (birth certificate, shot record, parent picture ID, proof of address and last grade report/transcript)
***Additional forms will need to be completed in person once all documentation is verified.
 
Returning Students - If you are a RETURNING Student, please complete the following forms:
- Returning Student Enrollment Form
- Student Service Questionnaire
- Housing Questionnaire
- Document Upload 2024-2025 for any documents for your student's file (birth certificate, shot record, parent picture ID, proof of address)
 
If you have a NEW SIBLING enrolling, please complete all the New Student Enrollment Forms for the new sibling.
